Mower bushing

Anyone ever have the lift shaft bushing come off from behind the cam base at the rear linkage of the mower deck? This is on the left side if sitting on tractor.I believe the bushing was installed from the outside of the cam base and just worked loose? I was lucky to see it in the grass before i ran over it----Looks like an easy fix.
It's a 60" deck on a b2920.

That happened on my b2620, seems there was a bit of an issue with these adjusters. They tended to ratchet down the cam on uneven ground, etc.

From my research it turns out that it was a top product issue listed in the kubota dealer's online service center (HERE is a link for the instructions to solve the problem)

On my b2620 they did the mod with the washer and grinding, but didn't put a locking nut on it as suggested in the document so it ended up vibrating off.

If I remember correctly it should be a M10x1.25 nut

After replacing the nut and snugging it up enough so that there wasn't any slop I haven't had any issues.

I called my dealer about this problem and they were embarrassed about it and ordered me a nut and chewed out the service guy that installed the deck, I had 1.2hrs on mine when if fell apart......

Thanks for the info-----On mine it's the bushing "120" in the schematic in your link. I cannot see how it could have come off unless it was installed from the outside?
